If you are injured playing hockey for the club, you may be able to claim for certain expenses or loss of income relating to your injury.

There is a section on the claim form which needs to be completed by the club secretary and treasurer. Please email secretary@waverleyhc.org.au and treasurer@waverleyhc.org.au and the club secretary and treasurer will sign and witness the form.
